ALL Medical and Nursing concerns must go through Primary RNs.

 

Contact for: questions on scheduling or notification of medical appointment, accompanying DDI staff on medical appointments, diagnoses, treatments prescribed or testing recommendations, follow ups required.

Additionally parental concerns surrounding weight, lethargy, medications, allergies, bowel disturbances, blood work or testing results etc. should all be directed to Primary RN not management or direct care staff.

 

RN SUPERVISOR

RN Supervisor is responsible for supervising the Primary RNs and the Medical Transport Assistants. She is responsible for ensuring that all annual nursing care plans are implemented as prescribed by doctor as well as the creation and implementation of CRP/DDI/OMRDD medical and nursing regulations and guidelines.

Liaison for CRP for Opti Healthcare and other DDI divisions, Medical Director, Shore Pharmacy, outside DDI medical facilities and DDI’s Behavior and Medication Review Committees.

Contact for: After consulting with Primary RN, still feel uncomfortable or that situation is not resolved, still have questions, etc, then RN Supervisor may be able to help.  

APPLIED BEHAVIOR SPECIALISTS (ABS’S)

Responsible for the development and implementation of Behavior Support Plans (BSPs)  for students on their caseloads by conducting functional analysis via staff interview, data summary and observations.  Responsible for training all staff on each student’s BSP. Liaison with families for BSP consents, recommendations for psychotropic medications from psychiatrist/IDT, revisions in BSPs.

Contact for:  questions and concerns with dramatic changes of behavior, emergence of new problem behavior, questions about psychiatrist recommendations for psychotropic medications, and discussions about proactive and reactive strategies in BSP’s which may include SCIP-R, protective equipment etc.

SPEECH INSTRUCTORS

Responsible for the development and implementation of communication programs for students on their caseloads by conducting functional analysis via staff interview, data summary and observations. Responsible for the creation and maintenance of augmentative communication devices (PECs symbols, books, devices). Acts as liaison between student’s educational settings for communication programs.

Responsible for training all staff on each student’s communication programs.

PSYCHOLOGICAL COORDINATOR

Responsible for ensuring that all BSPs and Communication programs are developed and implemented according to DDI and OMRDD regulation and philosophies. Conducts quality assurance checks on BSPs and Communication programs to ensure accurate data collection methods as well as ensuring they meet best practice strategies for positive behavioral support.

Additional support at IDT/Quarterly meetings in the assessment of problem behaviors and development of strategies to address problem behaviors.

SOCIAL WORKER

Responsible for ensuring all Guardianship information to families; processing, private insurance bills, ensuring Medicaid eligibility and funding for all CRP students; acts as Representative Payee for personal finances of students in CRP; processes referrals for all incoming and outgoing CRP students; conducts tours and screenings for potential CRP students; responsible for ensuring smooth transition services for all incoming and outgoing CRP students.

Instructional Manager

Responsible for ensuring supervision of assistant residence program managers and community instructors, ensuring that each program implements and maintains DDI policies and procedures, interfaces with families of their involvement in their child’s program plan, daily life, advocates for the individuals’ supported and implements new programming ideas, aids in the creation of staff schedules , running staff meetings, runs the shift runner course, and management exercises within the residence and CRP program. Responsible for hiring, firing, progressive discipline and performance evaluation of staff and knowledge and participation in all budgetary processes for the residences.

PROGRAM COORDINATOR

Responsible for ensuring across all clinical areas and educational setting, that each student’s annual program plan is developed, written and implemented according to CRP, DDI and OMRDD regulations, guidelines and philosophies (includes social work, psychology, nursing/medical, communication, academic, vocational, self care, independent living, life safety, nutritional). Conducts quality assurance checks, IDT meetings, observations, attends quarterlies and annual reviews as needed.

Contact: After Residence Management team for household issues such as student supervision, data collection issues, clothing/furniture inventory, general dissatisfaction with staffing/management issues, lack of progress on goal etc.

Contact: After ABS/Speech/Psych Coordinator, RN/RN Supervisor, Social Worker, for additional concerns or questions about BSP, Communication Programs, Annual Nursing Care Plans, transition/referral.

DIRECTOR CRP

Responsible for administrative management of all CRP staff including hiring, firing and termination for all CRP staff and management trainings; ensures compliance with all State, Federal, OMRDD and DDI policies; oversight of Operational Checklist and all budgetary processes for CRP; oversees investigation into allegations or serious reportable occurrences and ensures all recommendations from investigations are completed; ensures appropriate Incident Reports from each residence; ensures individuals are integrated into the community and oversees transportation issues.

Responsible for ensuring compliance with all State, Federal, OMRDD and DDI policies for the support of all students through direct supervision of Program Coordinators, Instructional Manager, Psych Coordinator, RN Supervisor, Staff Trainer, Social Worker, Cook Housekeeper Manager, Facility Maintenance Manager, Overnight Managers and Assistant Director.

Responsible for all fiscal and budgetary resources, development and implementation of CQI goals, CRP representative on agency-wide committees, development and implementation of CRP’s Guide to Children’s Services, staff training and development/enhancement.
